FSP Case Manager (LB) Job Details Job Location Long Beach Location - Long Beach, CA Description SUMMARY OF POSITION: The FSP Case Manager (CM) will work with clients and their families to provide case management services, collateral support, psychotherapeutic education, and behavior interventions. The CM will collaborate with the treatment team to ensure the family is linked to resources applicable to their needs and relevant to their goals. JOB RESPONSIBILITIES: ADMINISTRATIVE Adhere to company policies and procedures to ensure legal, ethical and safety standards are maintained. Comply with all regulatory requirements for professional practice and documentation. Collaborate with treatment team and applicable outside agencies to promote progress toward treatment goals and family needs. Ensure that individual billing and productivity objectives are met on a weekly basis. Complete all documentation requirements according to DMH guidelines and CB policies of 24 hours from date of service. Attend staff meetings and training sessions to enhance knowledge and skills relevant to position and to comply with company policies and state regulations Complete the Family Needs Assessment document, which identifies social, emotional, and psychological needs and strengths, and track progress toward satisfying the treatment-related needs of the client and family. ADVOCACY CM will collaborate with interdisciplinary team to promote and protect client and familys rights and safety. CM will support mandated reporting laws through clear communication regarding state and federal laws, education on developmental stages, and teaching appropriate disciplinary interventions. CM will model and encourage proactive self-advocacy through use of links to resources applicable to ongoing family needs and assertive communication BEHAVIORAL Contribute to assessing, planning, and implementing Client Treatment Plan (CTP). Deliver timely entries into client charts including (but not limited to) pertinent assessment and intake information, treatment goals and progress, documentation regarding supplemental services, and all progress notes. Meet with client, family, treatment team, school staff, and others involved in clients care to evaluate ongoing needs and services for completion of quarterly and annual assessments and revised treatment plans. Participate in trainings specific to case management, collateral support, rehabilitation services, and documentation. Provide behavioral interventions, behavioral and developmental education, and resource linkage to assist clients and families with case management needs and/or therapeutic goals. Collaborate with community based providers to assure continuity of care. Develop and participate in facility-based activities, such as support groups, parenting skill building, and family enhancement programs. Mobilize resources specific to clients needs including consultation and intervention with social security, welfare, housing, health departments, and other community agencies relevant to family needs. Participate in individual supervision sessions. DISCHARGE PLANNING Collaborate with family and treatment team to determine progress toward goals and readiness for discharge from program. Provide input to therapist regarding familys accomplishments, as well as ongoing needs, to insure development of appropriate discharge and referral plan. Provide termination session(s) to review and reinforce resources related to parenting support and rehabilitation service. EDUCATION Participate in new employee orientation and training, specific to case management duties and responsibilities. Attend and participate in facility and community trainings to enhance professional development. Engage with colleagues and supervisors in team building activities to develop and build personal and professional skills relevant to job duties and requirements. GENERAL Maintains 65% billable service delivery (26/wk) Monitors and ensures service utilization is commiserate with the level of client need and medical necessity. Ensure timely client access to services such that clients are identified, assigned, evaluated and treated within agency timeframe Other duties as assigned As a mandated reporter, must follow child abuse reporting guidelines and report all suspicions of child abuse and/or neglect. Qualifications JOB QUALIFICATION STANDARDS Experience Three years of direct service experience treating emotionally disturbed children and transitional-aged youth. One (1) year case management experience preferred Education, Professional Training, Technical Training or Certification This position requires a Bachelor's degree in Psychology, Sociology, or Human Services field; two years of experience in the social services may replace degree.CM must hold a valid drivers license, maintain current car insurance, and have the ability to travel in the field the majority of the work day. Knowledge/Skills This position requires that the employee is bilingual and able to read, write, speak and understand the spoken English and Spanish language to ensure the safety and wellbeing of clients when responding to their clinical needs. Maintains confidentiality in matters of agency operations, personnel, and clients Maintains positive communication with all agency staff, clients and visitors Exercises good judgement in performance of duties and responsibilities Supports existing agency policies, principles and mission. Knowledge of accepted standards of practice for case management services. Competent in industry values, ethics, policies, and laws. Ability to be flexible, independent, manage crises, and possess strong leadership skills. Awareness of emotional and cognitive needs and challenges experienced by mental health client and family. Ability to work cooperatively and effectively as a team member. Demonstrate solid verbal and written communication skills. Adept in problem solving and conflict resolution. The employee is required to sit, stoop, kneel and crouch. The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
