DescriptionPOSITION TITLE: PGH Health Services CoordinatorREPORTS TO: PGH AdministratorSUPERVISES: NoneFLSA STATUS: Non-exemptFTE: SALARY: DOEThis position outlines additional duties of DCS position1. Position Summary. The health services coordinator is responsible to the Administrator for coordinating health care for the residents and managing medication administration. 2. Prerequisites. The health services coordinator is considered DCS and must meet the prerequisites and be able to perform the duties contained in the DCS job description.3. Qualifications. Possess ability toEstablish effective working relationships with county mental health, doctors' offices, hospitals, pharmacies, clients, house nurse(s), and medication aids.Act independently.Problem-solve complex issues by developing alternatives and anize and establish priorities.Negotiate conflicts and resolve problems.4. Experience and Training. At least two years-experience providing direct support services, preferably to chronically mentally ill clients and experience with medication administration. 5. Physical Demands. This position will require on-call status in addition to those physical demands contained in the DCS job description. 6. Essential Duties and ResponsibilitiesGeneral Carry out all duties and expectations of your supervisor.Implement and carry out all applicable duties contained in the Health Care Services & Medication Management manual.Report and chart (if applicable) all incidents, illnesses of residents, results of doctor appointments, medication reviews, assigned tasks not completed and any other pertinent information to the administrator and nurse(s).Apprise and advise administrator and house nurse(s) of non-compliance issues relating to medication aids.Assist house nurse(s) in the performance of administrative duties in their absence as determined by discussion and collaboration with supervisor prior to departure.Access nursing assistance in the absence of the house nurse(s).Perform the duties contained in the DCS job description as required.Request time off (paid and unpaid) from supervisor in accordance with policy and procedures.Be available after hours for on-call consultation for QuickMar questions. Relay all medication questions to the nurse(s). Track your time spent away from Pioneer Guest Home but conducting business for Pioneer Guest Home and submit to the Assistant Administrator in a timely manner for payroll purposes. Administration Review, take action on, and initial all health related correspondence coming into the office.Develop and maintain an orderly filing system with coordination of supervisor. File health related correspondence once it is initialed by self and house nurse(s).Participate in client in-take procedures by obtaining and disseminating Releases of Information and obtaining medical card(s). File documents in client file.Participate in client discharge procedures in accordance with the Discharge Checklist.Responsible for Section 3 of client files.When communicating via MyChart, ensure your correspondence contains your signature block.Implement and maintain the QuickMAR system to fullest potential. Medication Management Set up the medication cart for the month and for clients when they are out of the facility.With the help of the nurse(s), transmit physician's orders to the pharmacy and check the pharmacy's automated physician's orders for accuracy and correct information when they come in for the new month, or as updates occur.Work the new cycle-fill in accordance with policies and procedures to ensure all medications are in and correct.Identify and order new medication from the pharmacy and keep at least a five day supply of medication in the medication cart and cabinet by conducting an inventory no later than noon Thursday each week.Obtain a written physician's order for each medication prescribed to a resident.Collaborate, communicate, and coordinate with medication aids to ensure physician's orders are carried out. If you direct medication aids to deviate from medication administration protocols, instruct them to make an annotation of such in the QuickMar files. You must exercise highest judgment in these instances as you will be held responsible if your judgment is in error.Clean the medication cart, cabinet, refrigerator and medication room on a weekly basis.Assist in destruction of medication in accordance with house policy and procedure.The registered nurse(s) is responsible for selecting medical supplies. Coordinate with nurse(s) and ensure adequate stock of those supplies on premises. Training of Medication Aids Assist house nurse(s) with training employees designated by the assistant administrator for delegation of medication administration by the house nurse(s). Delegation of actual training is prohibited under OR/OSBN rules.Reinforce principles taught through the delegation process by the house nurse(s). Day-to-day activities Transport clients to appointments both in and out of town. Coordinate with assistant administrator to ensure adequate drivers are available if conflicts in scheduling arise.Assist medication aids in administering medication, charting MARs/PRN usage, etc. by passing on medication related the absence of the house nurse, instruct medication aids on what various medication is used for.Review MARs from previous shifts to ascertain compliance with physician's put scripts into QuickMAR as required.Carry out delegation duties as assigned in writing by nurse(s). Client care Assist and accompany clients with psychiatric, medical and dental appointments.Have knowledge of the medical history of each resident with an accepting and understanding attitude.Responsible for making appointments, recording on calendar, and accompanying residents to labs, injections, etc.Responsible for making appointment, recording on calendar, and attending medication reviews.Assist medication aids in training clients in self-administration of medication.7.
